Feedback Forms
Feedback Forms enable you to collect structured input from guests through custom, multi-question forms published directly to one or more of your locations. Forms are designed using a template-based editor and can be deployed to any combination of venues, shared via direct link, or distributed via QR code.
Step 1: Select a Template
Begin by selecting from one of the pre-built form templates, each designed for a common use case:
- Customer Satisfaction — Gather qualitative and quantitative feedback on the overall guest experience
- Event Registration — Collect sign-ups and relevant details from guests attending an upcoming event
- Booking Form — Accept reservation or appointment requests directly through the form
- Lead Capture — Collect contact details from prospective customers for follow-up by your team
Step 2: Build and Publish the Form
Once a template is selected, the form editor provides full control over its structure and content:
- Add a welcome screen to introduce the form to respondents and set expectations.
- Add an end screen to acknowledge completion and optionally direct guests to a next step.
- Use the field picker to insert questions of the following types: text input, email, phone number, radio buttons, checkboxes, dropdown selection, date picker, star rating, or hyperlink.
- Each field can be individually configured with settings such as placeholder text, answer options, and whether the field is required.
- Fields can be renamed, duplicated, or reordered at any time during the build process.
When the form is ready for deployment, click Publish and select the locations at which it should go live. A form may be republished at any time to update the set of locations it is active at.
Step 3: Review Responses
Submitted responses are accessible through the Responses view associated with each form:
- Summary cards at the top of the Responses view display aggregate statistics, including total submission count.
- Filter responses by location or date range to focus on a specific subset of data.
- Customize the columns displayed in the response table to surface the fields most relevant to your analysis.
- Export the response dataset for further processing in external tools such as spreadsheet software or CRM systems.
Sharing a Published Form
For forms that are live at one or more locations, two sharing options are available directly from the form's entry in the list view:
- Direct Link — A shareable URL that opens the form in a browser.
- QR Code — A scannable code that can be printed or displayed at a venue to direct guests to the form.
